ROI Event Release and Health Declaration

I hereby grant to ROI Community (R.A.) (“ROI”), Charles and Lynn Schusterman Family Philanthropies, Charles and Lynn Schusterman Family Philanthropies – Israel (R.A.) (both “Schusterman”) as well as any other persons or entities acting on behalf of any of the above (“Authorized Recipients”) permission to use my name, my quotations, or photos or video footage of me (hereinafter: “Personal Materials”) in any materials produced by ROI, Schusterman, and/or Authorized Recipients in any media whatsoever, including but not limited to written materials, videos, social media, PR materials, etc. I agree to permit ROI, Schusterman and/or Authorized Recipients to use any and/or all of the Personal Materials produced, photographed and/or filmed within the event program at its discretion without compensation. I understand that the materials in which my Personal Materials may appear will not include my address and that this information will not be disclosed to third parties. I understand that this permission is perpetual in time. I hereby waive the opportunity or right to review or approve the finished Personal Materials, including but not limited to any photographs, video footage or quotations. I release and irrevocably waive any rights to the Personal Materials. I have read and understood the release stated above and agree to its terms and conditions.

Furthermore, with regard to my participation in any events organized by ROI, Schusterman and/or any of the Authorized Recipients, I declare that:

1. I will not arrive to the event if I am exhibiting one or more COVID-19 symptoms (such as fever, coughing, fatigue, loss of smell and/or taste, etc.). I hereby agree to cooperate with any reasonable steps event organizers may require in terms of maintaining health, such as having my temperature taken, filling in forms regarding my health and symptoms, leaving the event if I exhibit any symptoms associated with COVID-19, and in general agreeing to reasonable requests they may determine that are designed to ensure everyone is safe and feels safe.
    
2. I understand that I am responsible for my own health and safety. I waive any claims or complaints against ROI, Schusterman and/or any of the Authorized Recipients or those acting on their behalf in this regard.
